The first step you need to take is to visit their websites and download a consumer report request. Their policy allows consumers to get one free report per year. This report is going to be very useful because you will be able to see exactly what you are being reported for.

Second once you get your report you should get a reinvestigation form. Fill out the front side of the form and put the name of the bank, account number and amount, pretty much just follow the instructions. The backside (note: do not fill out the back where it asks for a customer statement. I have heard from many people that it is really admitting to the debt if you fill it out) So to play it safe don't fill it out.

I am hereby formally requesting a copy of any documents provided by [NAME OF BANK], bearing my signature, and showing that I have a legally binding contractual obligation to pay them. A bank clerk looking at their computer screen, seeing my name listed in their database is NOT verification or validation of any alleged debt.

Please note that I am making a goodwill attempt to have you clear up this matter. The listed item is entirely inaccurate and incomplete, and represents a very serious error in your reporting.

Failure to comply with Federal Regulations by credit reporting agencies is investigated by the Federal Trade Commission (15 USC 41, et seq.). I am maintaining a careful record of my communications with you for the purpose of filing a complaint with the FTC and the State of [YOUR STATE] Attorney General’s Office, should you provide noncompliance.
